a Queensland Cerebral Palsy Register member in two easy steps,
and help us gather information that will guide research in causes, prevention,
effective treatment and service provision of cerebral palsy. |

Step
1. Download
the PDF Consent Form to print out, fill in and send to the QCPR,
or contact the QCPR and
we will mail a copy of the Consent Form to you. Your registration
cannot be included on the register until your consent
is received by the QCPR. |

If
you have any difficulties with your registration, or questions
about registration, please do not hesitate to contact

information page including illustrated descriptions of each

Members will receive information and regular updates from the CP Register. |

| Information
collected by the QCPR is transferred into de-identified data*,
and then sent to the Australian Cerebral Palsy Register.
*De-identified data does not contain information which identifies individuals. |

| De-identified
data is also available from the QCPR to researchers, people with cerebral
palsy and their family/person responsible, policy formulators, the community
and health professionals. |

| All
outgoing data from the QCPR is de-identified. |

The
secure online
registration and database are protected by two firewalls
at both the Queensland and Australian level. |
